    Home hospice and palliative care

    Palliative care (supportive care) and home hospice is a medical support service, designed for patients with incurable diseases in advanced stages, who choose to stay at this period of their lives at home, surrounded by loved ones.
    Sabar Refuah operates a dedicated system nationwide that provides the full range of palliative care in the patient's home. The service is provided to patients of all ages, with full staff availability all year round, to provide care and support to the patient and family members. Home hospice service is provided by Sabar Medicine fully funded by the HMOs to the HMOs insured in the arrangement. Patients not included in the arrangement can receive the service privately.

    The palliative care is to assist terminally ill patients and their family members in the process of coping with an incurable disease in its advanced stages, and enable them to remain in their most natural and comfortable surroundings - their home. The purpose of palliative treatment (and the private home hospice service) is to improve the quality of life and comfort as far as possible, prevent suffering, ease the physical pain, anxiety and other symptoms caused by the illness such as: fatigue, respiratory difficulties, loss of appetite, sleeplessness etc.

    By law, home hospitalization in Israel is financed by the HMOs and is part of the basic rights of all citizens, especially those dealing with an incurable disease in its advanced stages, according to the Law of National Health Insurance. The Law for Patient Rights secures a professional, high quality and full-availability Home Hospital service for every patient, that is in no way inferior to that provided in hospitals. Home Hospital is provided with the choice and consent of the patient. The patient is surrounded by those closest to him, who look out for his interests and wellbeing, and the members of the household can continue with their daily routine.

    What does palliative care at home include?

    How does the service operate?

    Patients who are found to be suitable and that are entitled to Home Hospice will be referred to Sabar Health by a professional at the HMO or the hospital.Within 24 hours of the authorization of the referral to Sabar Health, a treatment team will meet at the patient’s home to meet and receive the patient

    If the patient is in the hospital, you should contact the HMO representative at the hospital and ask to come to the home hospice of Zabar Medicine. If the patient is at home, you should see your family doctor to get a referral to a home hospice. The referral will be forwarded by the doctor to the HMO’s home care unit, and through this unit you will be able to receive the referral to our service. It is important for you to know – the law in the State of Israel requires the health funds to provide palliative care that supports full availability 24/7 to every person who has a serious illness and is in the final stages of his life.

     

    In any case of distress, we are available to you 24/7 including on weekends and holidays. The nurses will leave you their mobile phone number and another phone number of standby during the weekend. For backup, we have a call center active 24/7 all year round for any need.
