    CHF Hospital At Home Ward

    The CHF Hospital At Home service was created based on the understanding that treatment of patients with Congestive Heart Failure (CHF) is highly complicated and therefore requires extensive cooperation between the treatment team accompanying the patient, the family and the community. The objectives of this ward are to extend their life expectancy whilst maintaining the best quality of life and comfort, and preventing or slowing down the progression of the disease, reducing hospital admisions and rational use of medicines and tests.



    How does the Home Cardiac Insufficiency service work?

    During Home Hospitalization for Cardiac Insufficiency, the patients are designated a permanent Sabar Health professional team that accompanies them and their families for the duration of the treatment period. The first visit will take place within 24 hours of acceptance of the service, and during that visit, the physician will perform a precise diagnosis of the patient and prepare a personalized treatment program for him. Staff members will conduct a prearranged home visits and, in accordance with the treatment program, will provide round the clock monitoring of the patient, and will always be available to him. The treatment includes the use of a unique technology which enables close monitoring and the real-time updating of the staff regarding the condition of the patient.

    Mobile ECG device

    Sabar Health is the only company that provides a mobile ECG service a part of the Home Hospitalization for Cardiac Insufficiency. This device is the only one of its kind approved for use by the the FDA. The mobile ECG identifies a wide range of conditions including arrhythmias, bradycardia, tachycardia and normal sinus rhythm, using a short, 30-second test. The test is performed using an electrode that transmits real-time data to the treatment team

     

    An user-friendly app for measuring and updating measurements

    The user-friendly app is installed on the mobile device of the the main care and the treatment staff, to enable immediate updating of the patient’s measurements, including blood pressure, body temperature, weight etc. The team is updated regarding all data in real time, and through the app it is possible to monitor the condition of the patient over time and pass on medical recommendations in the event of changes. The app also includes relevant information, contact options and medical recommendations
