    Sabar Health Home Rehabilitation service provides patients with a full and comprehensive alternative to institutional rehabilitation. Treatment in the department includes personalized accompaniment and rehabilitative treatment which takes place in the natural surroundings, the home. According to State Health Law, and like in every rehabilitation institute, the Sabar Health Home Rehabilitation department provides the patient and his family with a blanket of rehabilitation services for a variety of situations.

    Every patient in the Sabar Health Home Rehabilitation department is accompanied by a specialist rehabilitative physician who devises a personalized rehabilitative treatment program in accordance with his needs. Every patient is entitled to intensive treatment from teh specialist rehabilitative stage, available to him and his family, 24/7 all year round.


    Who is the Home Rehabilitation service for?

    Diagnoses suitable for Home Rehabilitation as an alternative to Institutional Rehabilitation:

    Any medical condition requiring rehabilitation that the patient chooses to undergo at home.

    Rehabilitation following COVID-19.

    Deconditioning following an acute deterioration of function following a prolonged hospital stay.

    Orthopedic rehabilitation following joint replacement surgery and additional orthopedic injuries.

    Neurological rehabilitation following a stroke (CVA) and other neurological diseases.


    Sabar Health Intensive Home Rehabilitation Program

    The department of Home Rehabilitation includes a multidiciplinary team from all branches of rehabilitation, that operates nationwide.

    • The treatment team comprises a permanent staff – a physician specializing in home rehabilitation, a nurse, a social worker, a physiotherapist and, where necessary, a speech therapist, an occupational therapist and a dietitian.
    • In the first stage, a visit will conducted by a specialist physician in order to devise an appropriate treatment program for the medical and rehabilitative needs of the patient.
    • The treatment program will define the treatment objectives, the individual medical professionals who will take care of the patient, and an organized visitation program.
    • The team is a permanent (regular) one, and is available to the patient and their family at all times, 24/7.
    • An updated evaluation – after stabilizing the medical condition of the patient, an updated evaluation will be performed in order to determine continued treatment.
     

    Advantages of Home Rehabilitation

    • Within 24 hours of receiving authorization, Home Rehabilitation treatment begins. Precious time is saved in waiting for a bed at rehabilitative institutes. It is important to be aware that the waiting time can be a hefty price to pay with there is a need for rehabilitation, since the sooner the process of rehabilitation occurs, the better the potential for rehabilitation of the patient.

     

    • The patient rehabilitates in his own home – in his natural and familiar surroundings, with his family at his side. During the rehabilitative process, the patient practises in the area where he will continue to live his live – in the passages of his home, the stairs, his own bath, etc. Practising in the natural surroundings permits adaptation and a more rapid return to routine.

     

    • Home Rehabilitation significantly reduces the risk of infections and other complications of an institutional admission.

     

    • Home Rehabilitation preserves the routine of the other family members and saves them both precious time and money in travelling to the rehabilitative institution, saves on lost work days, searching and paying for parking, etc.

    What do I need to do?

    • If you are about to be discharged from hospital, you must check whether “referred to continued institutional rehabilitation” or “referred to continued intensive home rehabilitation” is written on the discharge letter.
    • Prior to being discharged from the department, you must contact a HMO representative at the hospital and ask for the Sabar Health Home Rehabilitation service.
    • Upon receiveing the HMO authorisation and acceptance of the patient to this program, the sequence of treatment is fully preserved with the HMO for the entire duration of rehabilitation until it is complete.
